For treatment pre-authorisation, we will require the treatment guarantee form, which should be completed by your treating doctor. This needs to be emailed to us with all supporting information 5 days prior to the start of your treatment in order to receive approval swiftly. We will liaise with your treatment provider directly to arrange direct billing for your peace of mind.

For reimbursement claims, please download and complete our claim form and email this to us at together with all supporting documents.

We are able to accept pdf documents, scanned copies and photographs of your medical reports, invoices and receipts. However, please do keep your originals in case they are required at a later stage to verify eligibility of your claim and treatment costs.
